Notes
- All veterinary hospitals must have a superintendent who is a veterinarian
- If you are applying for a hospital licence, transferring a hospital licence or if the current superintendent leaves the hospital or a new superintendent is appointed, you must submit an Appointment of a Superintendent Form
- All current licence holders must sign the Appointment of a Superintendent Form
- Please allow 7-10 days for processing
- After processing:
- Your hospital licence information is updated on our online Hospital Register
- A letter of acknowledgement will be sent to the new Superintendent together with information regarding the duties of the superintendent and the Checklist for Hospital Inspections
