Notes
- All current licence holders must sign the application
- If a licence holder is deceased please contact the Board
- Please ensure the Board receives your application at least 14 days before the next Board meeting
- If the Board approves the change of name application:
- Your hospital licence information is updated on our online Hospital Register
- You will be sent a new Veterinary Hospital Licence certificate
